New Patient Information
- What to expect
- Range of Motion and Posture exam
- Height, Weight, and other necessary vitals
- Physical exam of joints including stress tests, muscle tests, and nerve reactivity tests
- Required information
- ID, insurance card if you wish to be billed for out of network care
- New patient paperwork – we will e-mail it to you, but if you wish to fill it out in office please arrive at least 15 minutes before your scheduled appointment
- Cost
- We are required to offer a good faith estimate of cost – if you would like a paper copy please ask the front desk
- For cash only patients the cost is $85 for the first visit and $50 there after
- For insurance cost is dependent on out of network benefits
- Medicare:
- Initial exam not covered by Medicare: $80
- Adjustments: 80% of Medicare rate is covered for out of network chiropractors
- Remaining may be covered by a secondary insurance
- Additional exams required every 6-8 weeks to meet Medicare standard of care, cost of exam not covered by Medicare, current rate is $45
- Medi-Cal
- Medi-Cal does not cover chiropractic but a $5 discount is available on the cash price
- All other insurance
- Please contact your insurer to see how much they will cover for out of network chiropractic
- Some companies do not have any out of network coverage
- Medicare:
- If your insurance has no coverage you are ultimately responsible for payment for services rendered
- Payment at time of service is recommended, many insurance companies will reimburse the patient directly for out of network care
- EOB, superbills, and statements can be e-mailed or mailed directly to you
- Medicare will send payment to us at patient request, but the patient is still responsible for the remainder of the bill
